Acknowledgment of Risks
I, the undersigned parent/guardian of the named participant, acknowledge and understand that attending and participating in the activities offered at YASI Fun Club Pty Ltd involves potential risks and dangers, including for myself if attending a Stay & Play session. These include, but are not limited to, physical injury, emotional distress, and damage to personal property, resulting from actions, inactions, or negligence of others, accidental or otherwise.
Activities Description
YASI Fun Club Pty Ltd ‘Stay & Play’ may include, but are not limited to, interactive games, climbing structures, art creation, sensory play, and use of the facility’s equipment such as a slide into a ball pit, a climbing net, a rock-climbing wall, a two-level play area with a staircase and hand railings, and various interactive and immersive installations.
Stay & Play Sessions
I understand that these activities are intended to be engaging and stimulating, and that I may choose for my child not to participate in any activity.
Assumption of Risk
I voluntarily agree to assume all risks related to the participation of my child in the YASI Fun Club Pty Ltd activities. I also exempt and release YASI Fun Club Pty Ltd, its owners, employees, assistants, volunteers, and agents from any claim, demand, or liability for harm, injury, damage, or death which may arise from my child’s or my own participation in these activities.
Medical Authorization
In the event of an emergency, I give permission for YASI Fun Club Pty Ltd staff to provide immediate treatment as required and will accept their recommendations regarding referral for further medical care.
Media Release I expressly consent to the use of any photographs, videos, or audio recordings taken of my child during their participation in YASI Fun Club Pty Ltd activities for promotional purposes. This includes their use in advertising, marketing materials, social media posts, on the YASI Fun Club Pty Ltd website, and in printed materials such as brochures and flyers, across all media platforms worldwide. This consent is given without expectation of compensation and acknowledges that these images may be edited or altered for creative purposes. I understand this consent can be revoked at any time by submitting a written request to [email protected].
Stay & Play Sessions
Any use of my personal device to record audio / video of my child will not include any other attendees, in accordance with the Privacy Act 1988. I will seek permission from the parent/guardian of a child before taking any recordings that may include their child. I release YASI Fun Club Pty Ltd from all liability regarding the incidental or intentional recording of my child by other parents/guardians who attend the Stay & Play session.
Parent/Guardian Code of Conduct – Stay & Play Sessions
I agree to behave in a positive and supportive manner while attending these sessions with my child, to model fair play for all children who attend.
I agree to be responsible for my child and always supervise my child when undertaking activities, and to follow the direction of YASI Fun Club Pty Ltd Creative Facilitators regarding these activities, or the behaviour of my child. I agree to be involved in decision making when it comes to my child’s participation in all activities provided by YASI Fun Club Pty Ltd.
I agree that YASI Fun Club Pty Ltd Creative Facilitators should provide immediate First Aid administration to my child in the event of an emergency, and that it is my choice to proceed or not proceed with their referral for further medical care. In the event of an emergency, I will complete an incident report to the best of my ability, under the guidance of the YASI Fun Club Pty Ltd Creative Facilitators.
Acknowledgment of Policies and Procedures
I have read and agree to abide by all policies and procedures of YASI Fun Club Pty Ltd as they pertain to participant conduct and safety. I understand that failure to adhere to these policies and procedures may result in the termination of my child’s participation privileges.
